1. Family Waiver - per family (to include whole family on the actual waiver names)

• NOTE: If you have completed this for a prior school year, please complete a new one to cover this school year

5. Code of Conduct

• NOTE: We are also in the process of creating some Ground Rules specific to the campus

6. Dress Code

• Deeply Rooted Co-op Application (if not completed yet)

• Community Class Sign Up Form - per student

• Semester Invoice Due (before starting, TBD) - Deeply Rooted leadership will email invoice to you directly
